For patients who have risk factors for developing Barrett’s esophagus and early esophageal cancer (BE/EAC), Northwestern Medicine offers a dedicated high risk screening clinic and treatment program. Relying on gastroesophageal reflux disease (GERD) symptoms alone can miss a significant number of individuals at risk. Only around 5 percent of GERD sufferers ever develop Barrett’s esophagus. Moreover, approximately half the people diagnosed with Barrett’s ever complained of GERD symptoms at all. That’s why we look beyond the typical risk factors to get a much clearer assessment of an individual’s likelihood of developing the disorder.

Each patient seen in our center will have a personalized plan for reflux management, lifestyle changes, and endoscopic surveillance developed for them. Integral to this approach is the use of novel innovation in BE/EAC. Northwestern Medicine uses the latest in noninvasive tools for BE/EAC screening. The program also features state of the art tissue sampling tools such as the WATS-3D brush and innovation in risk stratification with TissueCypher® which ensures that patients at increased risk to develop EAC can be offered endoscopic eradication therapy.

Treatment Procedures

Program for Endoscopic Eradication Therapy for BE/EAC

We offer endoscopic eradication therapy (EET) to all patients with BE-related neoplasia. We can also provide the full spectrum of treatment options for BE, including:

  • Endoscopic resection
  • Radiofrequency ablation
  • Cryotherapy

Minimally Invasive Surgery and Robotic Esophagectomy for EAC

The Northwestern Medicine PREMIER Program uses state-of-the-art robotic technology to perform an entire esophagectomy through five finger-tip-sized incisions in the abdomen and a small incision on the left side of the neck. This enables patients to start eating by mouth the day after surgery, eliminating the need for feeding tubes. Patients typically experience less pain and discomfort, and most are discharged within three to five days.

Laparoscopic and Endoscopic Treatments for GERD

We provide a multidisciplinary approach to GERD management that’s tailored to each patient, from standard PPI therapy to surgical anti-reflux options, to endoscopic anti-reflux procedures, such as transoral incisionless fundoplication.

Benefits of Research

Our program is part of the TREAT-BE Consortium, one of the largest research enterprises in the world focused on improving treatment of Barrett’s esophagus and early esophageal cancer. Our previously published data highlights that Northwestern Medicine outcomes rank as some of the highest in the world for complete eradication of this disease and the lowest rates of recurrence after endoscopic eradication therapy.

Screening and Surveillance

High-Risk Screening Clinic for BE/EAC

Northwestern Medicine offers a dedicated high-risk screening clinic for patients with risk factors for BE/EAC. This clinic looks beyond typical risk factors and uses the lates noninvasive screening modalities, such as EsoCheck™, EsoGuard™ and transnasal endoscopy, for early detection. These techniques can identify BE and early EAC without the need for sedated endoscopy. The clinic also uses GERD management programs, reflux physiologic testing, weight loss and smoking cessation strategies to help lessen an individual’s level of risk.

Innovation in BE Screening and Surveillance

From incorporation of catheter- or string-based screening tests and magnetic capsule endoscopy to in-office trans nasal endoscopy, we strive to use the latest noninvasive tools for BE/EAC screening. The program also features state-of-the-art tissue-sampling tools, such as the WATS3D Brush, and innovation in risk stratification with TissueCypher® to help ensure that patients at a higher risk of EAC can be offered endoscopic eradication therapy. These tools allow us to offer longer surveillance intervals to patients at lower risk.

In this episode of Better Edge podcast, Sri Komanduri, MD, discusses the latest advances in the screening and management of Barrett's esophagus and esophageal cancer. He highlights the significance of risk stratification to enhance patient outcomes and minimize the incidence of interval cancers. Additionally, Dr. Komanduri emphasizes the importance of improving patient access to care and the role of advocacy organizations in supporting both patients and clinicians.
